Please provide the hospital with a copy of your pet's up-to-date comprehensive health and medical documents, including history of vaccinations/vaccine schedule and preventative care records. If you are unable to obtain these records, we are happy to procure them on your behalf. Call the hospital to learn more.

Also, please bring a stool sample with you to your pet’s annual wellness visit and/or if your pet is suffering from GI issues. If your pet is having urinary issues, we also recommend obtaining a urine sample, if possible. While these tasks may not be pleasant, they provide our veterinarians with valuable information about your pet’s state of health.
